Leanne Thomas, 43, died in the accident. She was from the local area and formerly named Carey. A serious two-vehicle crash occurred on Thursday evening in County Tyrone.
Three more people went to the hospital with serious injuries. A young child was among those hurt.
Emergency services responded to a crash scene involving a Ford Ka and a Nissan Note. The incident location was Old Loughrey Road in Cookstown. The crash occurred shortly before 5 PM on February 20.
NIAS sent many responders, including paramedics and emergency crews. A HART crew and ambulance officer also went, along with the Charity Air Ambulance with a HEMS crew aboard.

The woman who died was a passenger in the Ford Ka and sadly died at the scene. The drivers of both cars and a young child in the Nissan were also hurt and taken to the hospital with major injuries.
